Role Expectations & Summary:
Our Individuals living at Hawkmount are looking for a Community Disability Practitioner for their daily needs.
Your role will be: Community Disability Practitioner. The expected diagnoses for the Individuals are Dementia with severe developmental disabilities; we have acquired brain injury, blindness, and deafness. All Individuals are wheelchair bound. Individuals may display physical behaviour of concern, however, along with response plans for each individual, de-escalation trainings, trauma informed care practices, and Non-Violent Crisis Intervention trainings are all provided as tools to use for any incidents that may arise. A behavioural support plan will be provided to advance and achieve Individuals' goals. The group home expectations of this position are support with hygiene routines, meal preparation, house chores, getting groceries, and medication administration, lifting and assisting clients. Driving is a requirement for this position; therefore, you must be comfortable with driving in all quadrants of Calgary. This position will require driving and attending medical appointments, sporting activities such as swimming, etc.
About You:
You are compassionate, attentive, and passionate about supporting people with disabilities. You are comfortable with providing full personal and ostomy care, participating in social and recreational activities, advocating for the Individual in the community, and encouraging healthy living styles & social interactions. You have previous experience with G tube feeding and lifting/ assisting individuals with a wheelchair. You have a good knowledge of community events and activities that could be useful to integrate the Individuals. You are professional, take ownership, and are willing to follow the agency's policies and procedures. You are solution-driven, where you can identify the need for support for the Individuals' apprehensions/concerns. You are willing to take the time to understand and know your Individual and act/respond in their best interest while maintaining professional boundaries. You have previous experience working with Individuals with disabilities. You are willing to research, develop, and implement programs and initiatives for the growth of the Individuals. You can easily manage documentation responsibilities for the Individuals.
Schedule & Benefits:
This temporary position 40 hours/week at our "Hawkmount" group home is located in the Northwest (NW) area of Calgary. The starting pay rate for this position will be $21.19. This fixed schedule will be Tuesday - Friday, Sunday 3:00pm - 11:00pm with Monday and Saturday off. This temporary position will be conclude on July 31, 2025 with the possibility of becoming permanent.
Minimum Requirements:
- Minimum of a diploma or certification in the Human Services sector or Disability Sector
- Must provide a valid class 5 driver's license and have a reliable vehicle with a minimum $2,000,000 liability insurance (equivalencies will be considered, programs completed internationally may require a WES or IQAS assessment)
- Intermediate in computer skills and administration skills
- Provide medical, technological, environmental, physical, and behavioural support
- Minimum required training in Emergency First Aid & CPR Level A
Preferred Requirements:
- Previous knowledge and experience regarding Brain Injuries & Severe Developmental Disabilities would be an asset
- Previously trained and have knowledge of medication administration
- Previous experience with nursing is an asset
- Have taken relevant trainings such as Non-violent Crises Intervention, Abuse Prevention Response Protocol, and Positive Behavior Supports, etc.
- Previous experience in a group home setting would be an asset
